Was the tooth resorted with a crown after the root canal? If you did the root canal but never sealed the tooth properly, the bacteria seep in and cause an infection. The root canal could also have failed. They are not fool proof and very technique sensitive. They have to be done really well to have the best chance. That is why I stopped doing them over 20 years ago.
Possibly an infection which can happen when root canal work is done. The nerve is removed and in time the tooth dies.
